So I worked at this hotel. I'm leaving out details here because I like using them as a reference, but they annually hosted a very large anime convention. During one of these, said restaurant had to close for a day and a half. The reason why is because we were getting a lot of overflow of costumed anime fans. Some guy in a samurai suit was having bad luck at a bar, and when another customer started to make fun of him, HE ATTACKED THE GUY WITH A KATANA.

Yes, actually, that's exactly what I mean. From what the bartenders described, he was telling various women about the person he was supposed to be and for some reason, that wasn't working.

I hate to say it, but I don't think one exists. It was a good 5 or 6 years ago, before video phones were super common, and I guarantee the hotel wouldn't let the security tapes get out. Everyone desperately wanted this to go away. The bar/restaurant were closed the rest of the night and next day for some really hardcore cleaning.

No, there's only one ending in which he dies (the act one "Failure" ending), and that's from drunkenly falling off the roof. He has a heart attack in both of the blind girl's endings, but he wakes up in the hospital.
