Having had ingrown nails myself, probably about as bad as the ones you get from the nail itself after a while.
My doctor shot my toe up with novacaine, cut the nail right down the middle with scissors, and yanked the offending half out.
If you do have this and want to TRY letting the nail grow itself out of your flesh instead, you can try cutting a v-shaped notch in the nail (this assumes you get ingrown toenails because you have an arched nail) to let the edges come in a little as the nail grows.

I had one of these, it came back after being cut out and just had to grow itself out over time. Infection is not the worst part, it's knowing that a thousand years ago most outdoor wildlife would have killed you by now.
I've had a big toenail come off before. Thankfully it came back - I know someone whose root was ripped out and it just covered over in skin, never grew the nail back. I got lucky, I guess.

They can. They get deep in the skin on the side of your toe. The blood usually comes in if you stub your toe (even in your show), drop something on it, etc.
Yeah a set of jewelers screwdrivers works much better at digging out an ingrown toenail than any knife. Also take care of your damn feet. Look at the bastards once in a while. Much easier to remover a millimeter of nail that is growing wrong than half an inch.
On a side note my pinky toenails are nearly non-existent and grow strait up. When they start catching on socks I just grab a pair of pliers and pull them off. Problem solved for the next six months. Much like hair if you fuck your nails up they will just grow back.
