Holy shit, This is the first time I've seen one of these levels beaten without save states.

It's probably a pre-programmed run or the video has been edited.

I think as long as the emulator is recording the video, it can just keep throwing away everything from your last save state up to the one you're making. That's just a guess though. There must be those "tool assisted runs" out there without the disclaimer, and if they exist for any game, KMW would be the most likely.

What's more is that if your lives hit 0, nothing happens. They just keep going down.

Also the various graphical glitches and dissapearing objects that the designer really didn't even care about as long as it made the levels harder.
