There's also speculation that in real life he's a quiet, shy, straight guy, and the flamboyant behavior is just him maintaining and reinforcing the image that's been providing him fame and fortune for 30+ years. It certainly works either way.

I wish I could find the clip from the early-90s Howard Stern TV show where they put him in a "straight disguise" and sent him out on a blind date.
