Another wrote it into the anchor's script.
Another created the graphic.
Another saw the graphic before it aired.
Another authorized the graphic to be put on the air.
Another switched the graphic on.
Another's job was to specifically watch for any mistakes and take evasive action in the case of such a trainwreck (or planewreck?)
All of them could potentially be fired for letting this happen.
None of them thought "hrm, where have I heard these names before?"
